These services allow for a more successful reunification when a child is transitioning to or from foster care.  Face to face visits, documentation, and 24 hour on call crisis intervention are available.  This may also cover transition into foster care with intensive preplacement activity to ensure proper placement and appropriateness of TFC placement. Includes case management, transportation, FAPT, IEP, therapy sessions, and coordination of pre-placement visits.  Post-placement services may include advocating for educational, independent living, or vocational needs; assisting new custodian
with effective interventions, behavior interventions,
and treatment goals.
